Overview of documents per application
With this application you do not need to submit any documents with your application.
No longer registered?
Were you previously registered in the BIG register, but no longer? Then you do need to submit additional documents. You will receive a message about this after submitting your application
Certificate(s) of Current Professional Status (CCPS)
This is proof that no foreign restrictions of authority are applicable. You must submit a CCPS from the country in which you obtained your qualification and from the countries in which you attended additional work placements or medical internships, or worked in your profession, for longer than three months after you had obtained your qualification. You must submit a separate CCPS for each country.
A CCPS must not be older than three months and must be an original, or an authenticated copy. On the list of competent authorities you will find the foreign organisations known to us from which you can obtain a CCPS or relevant information.
You do not need to supply CCPS from the Netherlands. View examples of documents/supporting documents that you have to provide from abroad (in dutch).
An authenticated copy of a valid identity document
Important: all information on the document must be clearly visible. Do not send your original identity document to us. Read more about who may authenticate your copy.
A curriculum vitae (CV)
This is an overview of your education and work experience. The CV must contain details of all your work experience and education up to the time of this application. It is also important that you indicate the start and end dates of each course and period of work experience. In the case of your current work experience or courses you are currently following, you should enter today's date as the end date. You must be truthful when drawing up your CV.
A certified original diploma or a certified copy of the diploma
A certified copy is a paper copy of your diploma with an ink stamp and a signature or initials from your education, or from a notary from an EEA member state. Most programs issue one or more certified copies of the original diploma. Applying for a (new) certified copy of your diploma can take some time.

An authenticated copy of a description of the training programme
This is a clear and complete substantive description of the training programme you took during your professional training.
Any other certificates or diplomas
Think of other specializations or further education after your vocational training. You send the original or a certified copy. Read more about who can certify your copy.

A declaration (which may be from your employer) about your practical and/or professional experience (the original or an authenticated copy)
This must be an official, signed declaration (from your employer or another party) showing the exact work you did in which job and during which period. Read more about who may authenticate your copy.
You already have a BES exemption and you want to extend it. Were you previously in the BIG register, but no longer? Then you must submit additional documents. You will receive a message about this after submitting your application.
You must provide the following mandatory documents with your application:
- Certificate of Current Professional Status (CCPS) of all countries where you have worked or lived for more than three months. This document must not be older than three months. You do not need to supply a CCPS from the Netherlands.
- A recent curriculum vitae (CV).
- Do you also want an exemption for a specific area of expertise that was not previously included in your exemption? Then provide supporting documents for the assessment of this. Read more under 'Apply for an exemption for a specific area of expertise'.
If you want to apply for an exemption for a specific area of expertise without a registered specialism (Article 14 of the BIG Act) in the BIG register, you must provide evidence for the assessment of the area of expertise. The following documents are then required for the assessment:
- A copy of the specialist diploma, originally obtained by the institution that issued the certificate;
- A statement from the institution that issued the specialist diploma, that it is the competent authority for the country of issue (for diplomas issued outside Europe);
- The official training curriculum for the speciality, issued by the competent authority in the country where the training was completed. The document must show that it was valid for the period in which the training was followed;

An overview of the courses followed (chronologically and with the duration and content of the components of the course);
- A list of operations performed during the training (for surgeons);
- A statement of continuing education and training after completion of your training as a specialist
- Statement(s) about work performed after completion of the specialist training. This declaration must also contain substantive information on the nature of the work (for surgeons this must also be supplemented with an overview of executed operations).
- A curriculum vitae.
Translations of the documents
If your documents are in a language other than Dutch or English, you must add the document in the original language as well as in an original translation. The translation must be done by a sworn translator. If you send a copy of the translation, you must have it certified by the authority that issued the document or a notary established in an EEA member state.
Please note: the translator can not authenticate the translated original document. This can only be done by the institution that issued the document, or by a notary from an EEA member state.
